At the south area of the Molding Department:  On or about August 8, 2022, the emergency exit access was obstructed by palletized plastic sheets and other palletized material, restricting  employee movement through the exit access and preventing quick access to the exit.  b. At the east side area of the Molding Department: On or about August 8, 2022, the emergency exit access was obstructed by a compressed cylinder, wood working tables and boxes of scraps, restricting  employee movement through the exit access and preventing quick access to the exit.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.  At station 5 of the Molding Department:  On or about August 8, 2022, employees were exposed to struck-by hazards, in that, the unused portion of the blade on the JET 18-inch (Model: JWBS-18QT / S/N: 1203842) bandsaw was not guarded, exposing employees to a struck-by hazards created by the cutting  motion of the blade.

29 CFR  1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):   a:  At station 1 of the Molding Department:  On or about August 8, 2022, the powered cord of the Dayton Blower (Model 1TDT5) was not inspected for damage prior to the start of the shift, in that, the outer sheath cover was damaged and covered with electrical tape.  b. At station 1 of the Molding Department:  On or about August 8, 2022, the powered cord of the bandsaw motor was not inspected for damage prior to the start of the shift, in that, the outer sheath cover was damaged.
