At station 5 of the Molding Department: On or about August 8, 2022, employees were exposed to struck-by hazards, in that, the unused portion of the blade on the JET 18-inch (Model: JWBS-18QT / S/N: 1203842) bandsaw was not guarded, exposing employees to a struck-by hazards created by the cutting motion of the blade.

At station 1 of the Molding Department: On or about August 8, 2022, the powered cord of the bandsaw motor was not inspected for damage prior to the start of the shift, in that, the outer sheath cover was damaged.
