29 CFR  1926.451(e)(9)(i): The employer did not a  provide safe means of access for each employee erecting or dismantling a scaffold where the provision of safe access was feasible and did not create a greater hazard:   a) Job site, 1300 Gardner Avenue, Troy Hill, PA: On or about August 12, 2022, during scaffold dismantling activities, the employer did not ensure that employees were provided with a safe means of access/egress. An employee was observed climbing down the frames of a walkthrough scaffold system, exposed to a fall hazard of up to at least approximately 25 feet.

29 CFR  1926.451(g)(2): The employer did not ensure that a competent person determined the feasibility and safety of providing fall protection for employees erecting or dismantling supported scaffolds or did not provide and require the use of fall protection for employees erecting or dismantling supported scaffolds where the installation and use of such protection was feasible and did not create a greater hazard:    a) Job site, 1300 Gardner Avenue, Troy Hill, PA: On or about August 12, 2022, during scaffold dismantling activities, the employer did not ensure that fall protection was used where a fall hazard of up to at least approximately 25 feet existed.
