29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:  a) Job site, 1926 Morrell, Pittsburgh, PA: On or about August 11, 2022, the employer did not ensure that employees used appropriate fall protection systems/devices on the rear porch roof home where exposed to a fall hazard of up to at least approximately 10 feet.  b) Job site, 1926 Morrell, Pittsburgh, PA: On or about August 11, 2022, the employer did not ensure that employees used appropriate fall protection systems/devices on the front porch roof where exposed to a fall hazard of up to at least approximately 10 feet.

29 CFR  1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, 1926 Morrell, Pittsburgh, PA: On or about August 11, 2022, the employer did not ensure that a ladder used to gain access to/egress from the front porch roof was extended at least 3 feet above the landing surface.
