29 CFR 1910.151(b): Adequate first aid supplies were not readily available.  a) Main Bay: On or about July 20 2022, Ophthalmic Solution eyewash manufactured by Physicians Care was stored in the first aid kit for employee emergency use, but it expired on June 2021, exposing employees to biological hazards.  b) Main Bay: On or about July 20 2022, Antiseptic First Aid Spray manufactured by Safetec of America was stored in the first aid kit for employee emergency use, but it expired on February 2021, exposing employees to biological hazards.  c) Main Bay: On or about July 20 2022, Burn Spray manufactured by Safetec of America was stored in the first aid kit for employee emergency use, but it expired on January 2021, exposing employees to biological hazards.  d) Kitchen: On or about July 20 2022,  Ophthalmic Solution eyewash manufactured by Physicians Care was stored in the first aid kit for employee emergency use, but it expired on November 2021, exposing employees to biological hazards.  e) Kitchen: On or about July 20 2022,  Antiseptic Spray manufactured by Medi-First was stored in the first aid kit for employee emergency use, but it expired on July 2020, exposing employees to biological hazards.  f) Kitchen: On or about July 20 2022, Burn Spray manufactured by Medi-First was stored in the first aid kit for employee emergency use, but it expired on June 2020, exposing employees to biological hazards.

29 CFR 1960.67 As required by 29 CFR 1904.32, the Agency did not certify that he or she has examined the OSHA 300 log and that the annual summary was correct and complete:  29 CFR 1904.33(a) The employer did not save the OSHA 300 Log, the privacy case list (if one existed), the annual summary, and the OSHA 301 Incident Report Forms for five (5) years following the end of the calendar year that these records covered.   a) On or about July 20, 2022, the employer did not retain at the establishment OSHA 300 logs for fiscal years 2019, 2020, 2021, and 2022.  b) On or about July 20, 2022. the employer did not retain at the establishment OSHA 300A annual summary forms for fiscal years 2019.
