29 CFR  1910.120(q)(11)(i): Upon completion of the emergency response, it was determined that it was necessary to remove hazardous substances, health hazards and materials contaminated with them from the site of the incident, the employer conducting the clean-up did not meet all the requirements of 29 CFR 1910.120(b) through 29 CFR 1910.120(o):  a)  At the facility located at 1 Armco Drive Lyndora, PA 16045, on or about May 26, 2022 - The employer did not conduct air monitoring to identify any IDLH condition, exposure over permissible exposure limits or published exposure levels, exposure over a radioactive material's dose limits or other dangerous condition such as the presence of flammable atmospheres or oxygen-deficient environments.
