29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets: (a) This violation occurred on or about August 15, 2022, at the job site located at 1000 Concord Street, Forney, TX; where employees doing re-roofing work on a pre-existing commercial building, working, and walking around on the ground where there were overhead hazards and exposing employees to head injuries by not wearing hard hats.

29 CFR 1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) This violation occurred on or about August 15, 2022, at the job site located at 1000 Concord Street, Forney, TX; where employees were doing re-roofing on the flat roof of a pre-existing commercial building, where employees were using extension ladders that did not extend 3-feeet above the landing of the roof. Employees were exposed to a fall hazard.
