29 CFR  1926.303(c)(7): All abrasive wheels shall be closely inspected and ring-tested before mounting to ensure that they are free from cracks or defects:  (a) On or about August 12, 2022, at or near Buddy 1-27H and Domindgo 1-22H Well Site in Williams County in ND, the cut off disc used on the DeWalt DWE 43115 Angle Grinder was not inspected prior to use, the cut off disc was not rated for the grinder in use. This resulted in the failure and disintegration of the cut off disc exposing employees to flying debris, struck-by, and projectile hazards.
