29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section: (a) On or about August 12, 2022, at or near Buddy 1-27H and Domindgo 1-22H Well Site in Williams County in ND, employees were operating a DeWalt DWE 43115 Angle Grinder without a safety guard being equipped exposing employees to unsafe conditions, flying particles/debris, and struck by hazards.

29 CFR 1926.303(c)(7): All abrasive wheels shall be closely inspected and ring-tested before mounting to ensure that they are free from cracks or defects: (a) On or about August 12, 2022, at or near Buddy 1-27H and Domindgo 1-22H Well Site in Williams County in ND, the cut off disc used on the DeWalt DWE 43115 Angle Grinder was not inspected prior to use, the cut off disc was not rated for the grinder in use. This resulted in the failure and disintegration of the cut off disc exposing employees to flying debris, struck-by, and projectile hazards.
