29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  A.  Lima Truck Wash, Inc., worksite located at 1590 Findlay Road,  Lima, OH 45801:  On or about August 18, 2022,  In the wash bay #5, the employer did not ensure employees were protected from falls while performing cleaning activities on top of tank trailers.  Employees performing these activities slipped near the back edge of the crash box on left side of the trailer and was exposed to a fall of approximately 10 1/2 feet to a concrete surface below.
