29 CFR  1910.178(a)(4):  Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly:  Forklifts with modified attachments were not equipped with new Instruction plates, in the following instances:   a)   Loading Yard - Fork Truck FDB03-1240-0325 modified with fork extensions that exceed manufacturer's specifications on attached plate. b )  Loading Yard - Fork Truck D2-02727 modified with fork extensions that exceed manufacturer's specifications on attached plate. c)   Loading Yard - Fork Truck A390V11626T modified with forks that exceed manufacturer's specifications on attached plate.
