On or about August 23, 2022, at the address of 7556 Harris Rd Celina, OH; The employer failed to provide and require eye/ face protection for employees operating CNC machines where flying metal shavings were present. Operators utilizing 90 PSI compressed air to remove metal shavings from parts/machines were exposed to eye injuries.

29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Master Precision Machining. Dba EDC Machining. On or about August 23, 2022, at the address of 7556 Harris Rd Celina, OH; The employer failed to establish an energy control program consisting of energy control procedures, employee training, and period inspections prior to employees servicing equipment such as but not limited to CNC #10, #15, #16 and mills #1, #7, #12 where equipment could be reenergized. Employees accessing the point of operation to change tooling without the benefit of an energy control plan in place were exposed to amputation hazards.

Dba EDC Machining. On or about August 23, 2022, at the address of 7556 Harris Rd Celina, OH; The employer did not ensure that the point of operation of CNC #10 was adequately guarded, to keep employees from accessing the point of operation while CNC machine is in operation. The employer operated CNC without interlock protections. Operators without the benefit of door interlocks were exposed to amputation hazards when retrieving parts. b) Master Precision Machining. Dba EDC Machining. On or about August 23, 2022, at the address of 7556 Harris Rd Celina, OH; The employer did not ensure that the point of operation of CNC #15 was adequately guarded, to keep employees from accessing the point of operation while CNC machine is in operation. The employer removed interlock key from door and placed in the lock. Operators without the benefit of door interlocks were exposed to amputation hazards when retrieving parts. c) Master Precision Machining. Dba EDC Machining. On or about August 23, 2022, at the address of 7556 Harris Rd Celina, OH; The employer did not ensure that the point of operation of CNC #16 was adequately guarded, to keep employees from accessing the point of operation while CNC machine is in operation. The employer bypassed interlock with magnet. Operators without the benefit of door interlocks were exposed to amputation hazards when retrieving parts. d) Master Precision Machining. On or about August 23, 2022, at the address of 7556 Harris Rd Celina, OH; The employer did not ensure that the point of operation of CNC #12 was adequately guarded, to keep employees from accessing the point of operation while CNC machine is in operation. The employer removed interlock key from door and placed in the lock.
