The employer must ensure: 29 CFR 1910.22(a)(3): Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. a. On or about August 25, 2022, multiple extension cords were used to supply electricity to industrial fans at work areas, such as but not limited to, Line 4, thereby exposing employees walking and working along the line to trip hazards.

1910.305(g)(2)(ii): Flexible cords may be used only in continuous lengths without splice or tap. Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced. a. Located at 6390 Commerce Court, Groveport, Ohio: On or about August 25, 2022, multiple interconnected flexible cords were used in the B2B-CO2 area between emergency exit M203 and M204 in order to power two industrial fans, thereby exposing employees to electrical fire hazards.
