29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids, or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about August 25, 2022, employees without proper eye and face protection were using pneumatic nail guns to assemble wood pallets on the site's south side of the assembly shed.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips, and sparks. On or about August 25, 2022, on the south side of the site's nailing area: a) An employee loaded nails into the nailing machine while exposed to a fall into the point of operations of the chains and sprockets of the pallet conveyor. b) An employee was operating the far end of the pallet conveyor with an open guarding gate exposing the employee operations of the chains and sprockets of the pallet conveyor.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy. On or about August 25, 2022, on the west side site of the site an employee cutting wood materials, was using an electrical wall outlet that did not have an outlet cover. that was powering a miter saw.
