29 CFR  1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  Jobsite - On or about August 25, 2022, employees were using a 15-foot orchard ladder on concrete to paint a building and an employee fell approximately 10 feet when the ladder collapsed and suffered serious injuries requiring hospitalization.

29 CFR  1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  Jobsite - On or about August 25, 2022, employees engaged in painting from ladders were not provided with training that would enable them to recognize hazards or minimize hazards where they were using a ladder in a manner that it was not designed for.
