29 CFR  1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested:  a) Community Living Center - On or about August 24, 2022, all employees required to wear respirators were not annually fit tested.  A nursing assistant was last fit tested in March 2020.  The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 patients.  The Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.134(f)(2), which was contained in OSHA inspection number 1585258, citation number 1, item number 1 and was affirmed as a final order on 5/12/22, with respect to a workplace located at 3200 Vine St. Cincinnati, OH 45220.  The Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.134(f)(2), which was contained in OSHA inspection number 1550068, citation number 1, item number 1b and was affirmed as a final order on 1/19/22, with respect to a workplace located at 5000 South 5th Ave, Hines IL 60141.  The Department of Veterans Affairs was previously cited for a repeat violation of this occupational safety and health standard 29 CFR 1910.134(f)(2), which was contained in OSHA inspection number 1473501, citation number 1, item number 1 and was affirmed as a final order on 2/25/21, with respect to a workplace located at 4545 East 9th Ave. Office Building 1, Suite 010, Denver, CO 80220.  The Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.134(f)(2), which was contained in OSHA inspection number 1538854, citation number 1, item number 1b and was affirmed as a final order on 2/2/22, with respect to a workplace located at 200 Springs Rd. Bedford, MA 01730.
