29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.  At 98 Great Star Court, Saint Augustine Florida:  On or about August 17, 2022, on the 5:12 pitch roof, the employer exposed framing employees to 10-foot to 19-foot fall hazards in that they were not protected by a fall protection system while installing framing and roof sheathing boards.  b.  At 98 Great Star Court, Saint Augustine Florida:  On or about August 17, 2022, at the second-floor stairwell opening, employees were exposed to a 10-foot fall hazard from the unprotected edges, in that they were not protected by a fall protection system.

At 98 Great Star Court, Saint Augustine Florida:  On or about August 17, 2022, on the 5:12 pitch roof, the employer exposed framing employees to 10-foot to 19-foot fall hazards in that the retractable lanyards were not secured to the structure by an anchorage device or method capable of supporting 5,000 pounds per employee.

At 98 Great Star Court, Saint Augustine Florida:  On or about August 17, 2022, in the trusses for the 5:12 pitch roof, the employer exposed framing employees to 10-foot to 13-foot fall hazards in that the the employees were not provided a ladder to access the roof from the second floor.
