29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from machines, flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  3917 Frank Scott Pkwy West, Belleville, IL 62223  On or about 08/25/22, the employer did not ensure that eye protective equipment was used when employees was exposed to eye injury while using pneumatic nail guns and a Circular Saw to fabricate and install roof sheathing.

29 CFR 1926.501(b)(1): The employer did not ensure that each employee on a walking/working surface with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  3917 Frank Scott Pkwy West, Belleville, IL 62223:  On or about 08/25/2022, the employer did not protect employees from the hazard of falling twelve (12) feet.  Employees were fabricating and installing roof decking and sheathing on the roof with no guardrail systems, safety net systems, or personal fall arrest systems.
