(a) New residential construction located on Lot #9 at 119 Summit Pointe, Dallas, GA 30132, Stairwell between first and second floor: On or about 08/29/2022, the employer exposed employees to fall hazards of up to 10-feet, 4-inches to the lower concrete slab, in that the ladder used to access the second floor had bent and split rungs which compromised the strength of the ladder as this ladder was not removed from service or replaced before the start of the shift.
