At 1661 N. Richland Avenue, Deland, FL: On or about July 13, 2022, the employer exposed an employee to a struck-by hazard while repositioning a dump trailer.

Richland Avenue, Deland, FL: The employer failed to report a work-related fatality of an employee that occurred on July 13, 2022 to OSHA within eight (8) hours. The employer was aware of the fatality on July 13, 2022 at 7:11 pm.
