29 CFR 1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about September 1, 2022 at jobsite located in 1160 S Sandstone Dr. Coal City, IL 60416 , employees were exposed to eye injuries while engaged in roofing activities operating a pneumatic nail gun. Eye protection was not being utilized while employees engaged in this activity. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about September 1, 2022 at jobsite located in 1160 S Sandstone Dr. Coal City, IL 60416, employees were exposed to fall hazards in excess of 18 feet while engaged in residential roofing activities. Fall protection was not being utilized while performing roofing activities. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or about September 1, 2022, the employer did not ensure that all employees exposed to falls had been trained in the appropriate use of fall protection, such as, but not limited to the use of personal fall arrest systems. Multiple employees were exposed to falls of approximately 18 feet to the ground below, which could result in permanent disability or death. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
