The horn on the blue liquid propane-powered Mitsubishi forklift was found to be defective and was not  taken out of service until restored to safe operating condition.

29 CFR  1910.243(c)(1):Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4):  At the workplace located at 307 Craig Street, Hopkins, Missouri, 64461, employees were exposed to lacerations and struck-by hazards. Employees were operating a DeWALT DWE402 angle grinder in the workplace without a 180 degree guard that protected hands from the cutting wheel.

29 CFR  1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation:  At the workplace located at 307 Craig Street, Hopkins, Missouri, 64461, employees were exposed to electrical shock and burn hazards. A flexible extension cord was observed to be in disrepair with damaged or missing outer insulation, and abrasions, that were repaired with electrical tape.
