29 CFR  1926.451(g)(1)(iii):Each employee on a crawling board (chicken ladder) shall be protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board:  Site - On or about August 30, 2022, employees were working from a chicken ladder scaffold and were not utilizing adequate fall protection in either a fall arrest sytem, guardrail system or grabline.

29 CFR  1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Site - September 20, 2022 - Employees were not adequately protected from falls in that the fall protection was not adequately anchored for employees working on the church.
