29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems:  a)  At Meadow Loop West Rd Lot A-192 Semmes, AL  (Roof Area):  On or about August 29, 2022, the employer exposed employees to fall hazards up to approximately  sixteen(16) feet to the ground below, in that fall protection was not utilized by employees performing roofing work.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a)   Meadow Loop West Rd Lot A-192, Semmes, Alabama  (Roof Area):  On or about August 29, 2022, and at times prior; the employer  failed to produce a written (training) certification record for employees performing roofing activities exposed to fall hazards up to approximately sixteen (16) feet to the ground below.
