29 CFR 1926.451(e)(5)(i):Ramps and walkways 6 feet (1.8 m) or more above lower levels did not have guardrail systems that comply with subpart M of this part . a) On or about September 6, 2022, employees were exposed to fall hazards in excess of 13 feet when using a single scaffold plank as a ramp connecting two sections of scaffold. No guardrails or any other fall protection systems were in place.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) On or about September 6, 2022, employees were exposed to fall hazards in excess of 12 feet where no fall protection was in place when engaged in stucco operations on outside ledges of the building.

29 CFR 1926.1053(b)(4):Ladders shall be used only for the purpose for which they were designed. a) On or about September 6. 2022, employees were exposed to fall hazards where a portable step ladder used to access an elevated work level was not used in accordance with the manufacturer's guidelines. The ladder was in a closed position and used with the spreaders not locked in place.
