29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   a)  At 9058 Church Street Semmes, AL (Roof Area/Building 1) :  On or about August 31, 2022, and at times prior; the employer exposed employees to struck-by hazards, in that eye protection was not utilized by employees operating a  pneumatic nail gun during roofing work.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) At 9058 Church Street Semmes, AL (Roof Area):  On or about August 31, 2022, and at times prior; the employer failed to produce a written (training) certification record for employees performing roofing activities exposed to fall hazards up to approximately seventeen (17) feet to the ground below.
