29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  Location: 105 High Road, Newbury, MA 01951  On or about 09/07/22, Depiero, LLC, as a controlling employer did not ensure that frequent and regular inspections of the job site were performed by a competent person. As a result, subcontractor employees were exposed to fall and struck-by hazards in the following instances:  Instance a: Employees of NF Siding, Inc., were exposed to fall hazards of 10 feet or greater while installing fascia boards to the roof edges.   Instance b: Employees of Olivaries Construction, LLC, were exposed to struck-by hazards from an overhead suspended load of granite curbing hoisted by a modified excavator.   Instance c: Employees from S&R Construction, Inc., were instructed by the foreman of Depiero, LLC, to operate a Lull without proper training or certification.
