1926.21(b)(2)  The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  a)  Jobsite, Haller Street at N 4th Street, Youngwood, PA 15697:  On or about September 6, 2022, an employee working at the bottom of an excavation was not protected from  cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).   The excavation was approximately 9 feet 5 inches in depth with vertical sidewalls.  The employee was exposed to an engulfment hazard.
