29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:  a)  PRO USA Home Improvement does not ensure that employees use protective helmets where there is a possibility of head injury from impact, or from falling or flying objects, or from electrical shock and burns. This violation was most recently observed, on or about 9/8/22, at 491 Passaic Street, Passaic NJ, NJ,  Employees were exposed to a head injury while employees were engaged in roofing work above. The employer did not ensure that employees working below roofing work were protected by protective helmets, thus exposing employees to a struck by hazard to the head.  b)  PRO USA Home Improvement does not ensure that employees use protective helmets where there is a possibility of head injury from impact, or from falling or flying objects, or from electrical shock and burns. This violation was most recently observed, on or about 9/8/22, at 491 Passaic Street, Passaic NJ, NJ, . Employees were working on the lower roof while two other employees were working right above three employees.

Employees were nailing shingles with a pneumatic nail gun without the use of eye and face protection.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels is not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a) PRO USA Home Improvement does not ensure that employees use appropriate fall protection while engaged in residential construction activities 6 feet (1.8 m) or more above lower levels. This violation was most recently observed, on or about 9/8/22, at 491 Passaic Street  NJ.  Employees were exposed to a 12 to 25 feet fall hazard while not utilizing fall protection, employees where installing shingles without the use of personal fall arrest system or other means of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)Building front:  PRO USA Home Improvement does not ensure that where portable ladders that were used to access an upper landing surface, the ladder side rails extend at least 3 feet above the upper landing surface that was being accessed, on or about 9/8/22. Employees were exposed to 12 foot fall hazard while transitioning from the ladder to the roof.
