29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) 23 South Summit Street, Bergenfield, NJ (Exterior Front): An employee engaged in the disposal of construction debris on the ground level, was exposed to a struck by hazard from falling material. The employer did not ensure that employees were protected by protective helmets when there was a possible danger of head injury from falling objects, on or about 9/10/22.

29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) 23 South Summit Street (Exterior Front): Employees engaged in roofing work were using hand-held pneumatic nail guns, without the use of eye and face protection. The employer did not ensure that employees were protected with eye and face protection, thus exposing employees to an eye injury, on or about 9/10/22.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 23 South Summit Street, Bergenfield, NJ ( Exterior Front): Employees engaged in roofing work were exposed to a fall hazard of approximately 15 feet above the lower surface, without the use of a personal fall arrest system. The employer did not ensure that employees performing roofing work were provided with a personal fall arrest system, or other means of fall protection, thus exposing employees to a fall hazard, on or about 9/10/22.  CH Brothers Home Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1269986, citation number 1, item number 1 and was affirmed as a final order on 01/03/18, with respect to a workplace located at, 27 Dover-Milton Road, Oak Ridge, NJ 07438.
