29 CFR  1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   (a)Wilson Carranza at 8207 Monroe Place, Sioux Falls, SD 57106: On or about and at times prior to September 12, 2022, the exposing employer did not ensure an accident prevention program was developed and implemented for the work environment, to include regular inspections on the jobsite, evaluation of materials, tools by a competent person.  This condition exposed employees to fall hazards and a lack of training.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  (a)Wilson Carranza at 8207 Monroe Place, Sioux Falls, SD 57106:  On or about and at times prior to September 12, 2022, the exposing employer did not ensure that employees were protected from fall hazards while conducting residential roofing operations at heights greater then six feet. This condition exposed employees to fall hazards greater than six feet.
