29 CFR 1926.453(b)(2)(v):  A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  At the construction site located at 1630 S Killingsworth Ave, Bolivar, Missouri:  Employees were exposed to fall hazards when using a JLG aerial lift without the use of a fall harness and lanyard.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At the construction site located at 1630 S Killingsworth Ave, Bolivar, Missouri:  Employees were exposed to fall hazards.  Employees were performing roofing activities on the structure at heights greater than 10 feet without the use of a fall protection system.
