Manor Lake Assisted Living located at 50 Conference Center Drive, Cartersville, GA 30121, Front side: On or about 09/15/2022, the employer exposed employees to severe lacerations/amputation hazards while cutting plywood sheathing with a Makita 7-1/4-inch circular saw, in that a wood shim was used to keep the lower guard from retracting immediately after a cut was made, thus by passing the safety guard.
