29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles: Manor Lake Assisted Living located at 50 Conference Center Drive, Cartersville, GA 30121, Roof: On or about 09/15/2022, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns, in that protective eyewear was not provided while operating these tools at 120 PSI. Employees were exposed to plastic collated fragments, wood debris, and 2-3/8-inch fasteners.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe. Manor Lake Assisted Living located at 50 Conference Center Drive, Cartersville, GA 30121, Front side: On or about 09/15/2022, the employer exposed employees to severe lacerations/amputation hazards while cutting plywood sheathing with a Makita 7-1/4-inch circular saw, in that a wood shim was used to keep the lower guard from retracting immediately after a cut was made, thus by passing the safety guard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: Manor Lake Assisted Living located at 50 Conference Center Drive, Cartersville, GA 30121, Roof: On or about 09/15/2022, the employer exposed employees to fall hazards while working a heights of 9-feet, 3-inches from the lower ground level, in that a fall protection systems was not provided to employees engaged in plywood sheathing installation on a 5/12 pitched roof.
