At the jobsite located at: 2692 W. Loula Street Olathe Kansas, employees engaged in residential roofing activities with the use of pneumatic nail guns were exposed to struck-by hazards. Employees were not provided with eye protection.

Loula Street Olathe Kansas, employees engaged in residential roofing activities  were exposed to a fall distance of approximately 17 ft. from the roof to the ground below in that fall protection was not utilized.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the jobsites located at 2692 W. Loula Street Olathe, Kansas, employees engaged in residential framing activities were exposed to falls from approximately 17 feet from the roof to the ground below, in that the aluminum ladder used to access the roof did not extend  at least 3 feet above the eave of the roof.
