29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Jobsite located at 24th & Washington Avenue, Philadelphia, PA: The employer did not ensure that employees conducting roofing activities  on the roof of aa commercial building, approximately 24 feet from the ground level were protected from fall hazards by the use of a fall protection system. Condition was observed on or about September 15, 2022.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  a) Jobsite located at 24th & Washington Avenue, Philadelphia, PA: The employer did not ensure that the ladder side rails extended at least three feet above the landing surface. Workers were exposed to fall hazards of approximately 25 feet to the ground level.  Condition was observed on or about September 15, 2022.

29 CFR  1926.150(c)(1)(viii): Portable fire extinguishers were not inspected periodically and maintained in accordance with maintenance and use of portable fire extinguishers, N.F.P.A. No. 10A-1970:  a) Jobsite located at 24th & Washington Avenue, Philadelphia, PA: The employer did not ensure that the fire extinguisher such as, but not limited to Strike First portable fire extinguisher was inspected and maintained in a state of readiness for instant use.  The fire extinguisher was uncharged nor tagged to indicate last service date.     Condition was observed on or about September 15, 2022.
