29 CFR  1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment.  On or about September 1, 2022, and at times prior thereto, the employer did not conduct and certify a workplace hazard assessment to determine the procedures for employees guiding vehicles into the bay, exposing employees to the hazard of being struck-by moving vehicles and equipment.

29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.   On or about August 26, 2022, at approximately 9:20am, an employee suffered an injury that resulted in hospitalization. The hospitalization was not reported within 24 hours to the local Area Offices of the Occupational Safety and Health Administration.
