On or about September 13, 2022 at jobsite located in 11610 2nd St., Huntley, Il 60142, employees performing masonry activities were using a scaffold as a means of access to the upper level of a residential structure property. Employees were exposed to fall hazards greater than 10 feet from collapse of the scaffold caused by instability, likely to result in serious injury. Employees were exposed to these hazards when the employer failed to ensure supported scaffold poles, legs, posts, frames, and uprights bore on an adequate firm foundation.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
