29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)  a) Location - 102 Lynnfield Street, Peabody, MA:  On or about 09/15/2022,  the employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) to identify unsafe conditions such as, but not limited to:  multiple workers exposed to falls, ladders which did not extend 3' beyond the level to be accessed, fall protection anchor points not properly installed and multiple robe grabs attached to a single anchor point.
