However, the types of personal protective equipment, such as but not limited to heat and flame resistant jackets, sleeves and gloves for employees who conduct welding, were not required and used.

This powered industrial truck had unserviceable tires and a broken backup light. The deficiencies had been noted during the pre-shift inspection of the powered industrial truck.
