OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire, explosion, and deflagration hazards:  a) On or about 11/7/2022: 4022 Mill Road, Skaneateles, NY 13152:  Equipment such as, but not limited to Ross Ribbon Blender and All-Fill Auger Filler were being utilized to mix and dispense a powdered mixture of psyllium husk, citric acid, maltodextrin, and orange flavor.  The mixture was sampled and determined to be explosive.  The employer did not implement procedures for managing and/or mitigating fire and explosion hazards of combustible dusts or related particulate solids.  Deficiencies include but are not limited to:         a)    Communication of hazards and responsibilities to employees;     b)    Dust Hazard Analysis (DHA) not performed;     c)    Controls not in place to prevent sources of ignition such as but not limited to static electricity, sparks and heat;  Abatement documentation is required for this item.
