29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: a) Jobsite located at or near 2200 W Clearfield St, Philadelphia, PA - On or about September 14, 2022, the employer did not ensure that the trench approximately 14 feet wide, by 12.5 feet long, by 11.5 feet deep was inspected by a competent person at the beginning of the day and throughout the shift as necessary. Employees were working in a trench without the use of shoring, thereby exposing employees to cave-in hazards.

29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) Jobsite located at or near 2200 W Clearfield St, Philadelphia, PA - On or about September 14, 2022, the employer did not ensure that each employee in a trench approximately 14 feet wide, by 12.5 feet long, by 11.5 feet deep was protected from cave-ins by an adequate protective system. Employees were performing pipe repair in a trench without the use of shoring or other protective system, thereby exposing employees to cave-in hazards.
