29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:  a) 225 West  Loockerman Street, Dover, Delaware: On or about September 12, 2022, the employees were not provided training which would allow them to recognize and avoid hazards specific to the site and applicable standards on the site where they conducted framing operations at heights of approximately 12 feet without being protected from falls and exposing them to multiple hazards including but not limited to access/ egress, fire protection/ prevention and medical treatment which was observed.

29 CFR 1926.50(d)(1): First aid supplies were not accessible to the employees at the site:   a) 225 West Loockerman Street, Dover, Delaware 19901 - North, West and rear side of the facility: On or about September 12, 2022, the employees worked with materials and equipment including but not limited to electric circular and table saws as well as lumber framing materials and associated fasteners that could cause injuries including but not limited to bleeding injuries without the availability of a medical kit to control bleeding onsite.

29 CFR 1926.150(a)(1): The employer did not develop and maintain a fire protection program to be followed throughout all phases of construction and demolition work, and did not provide for the firefighting equipment as specified in this subpart:   a) 225 West Loockerman Street, Dover, Delaware - North, West and rear side of the facility: On Sept 12, 2022, CSHO observed employees working with electric circular and table saws at heights of approximately 10 feet without a fire protection plan including fire protection devices that would allow for safe egress such as but not limited to fire extinguishers as well as safe egress and accountability.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 225 West Loockerman Street, Dover, DE - North, West and rear side of the facility: On or about September 12,2022, CSHO observed three employees exposed to a fall hazard of approximately 10 feet above a paved surface without any means of fall protection while they performed framing operations on the second story addition.
