29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a)Triple M Framing, LLC at E Painted Sky St:  On or before September, 19 2022 the creating, correcting, and exposing employer did not protect employees from injury in that employees were working to install sheathing on the roof of a 2 story home, at 18 feet to point of exposure and 25 feet to the peak, without fall protection in place. This condition exposed employees to a fall hazard of 18 feet.   Abatement Note: Abatement certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."

29 CFR  1926.602(d):The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178.    (a) Triple M Framing, LLC at E Painted Sky St:  On or before September, 19 2022 the creating, correcting, and exposing employer did not protect employees from injury in that an employee operated a rough terrain forklift to move sheathing for employees on roof to access. The employee operating the forklift had not been trained and was not certified to operate the forklift. This condition exposed employees to struck by and caught between hazards from the forklift.  Abatement Note: Abatement certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."

29 CFR  1926.1053(b)(13): The top or top step of a stepladder was used as a step:  (a) Triple M Framing, LLC at E Painted Sky St:  On or before September, 19 2022 the creating, correcting, and exposing employer did not protect employees from injury in that employees utilized the top and top step of a 8 foot A-Frame ladder to access a 9.9 foot second story landing.  Abatement Note: Abatement certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."
