29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50 feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets.  On or about September 19, 2022, on the site's south side, employees were performing roofing on a low slope roof at a height greater than six feet without fall protection or outside of the warning line and without a safety monitor present.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On or about September 19, 2022, on the west side of the site, an employee used a ladder that did not reach the edge of the roof. The ladder's side rails were not extended at least three feet above the landing, and the ladder was not secured.
