29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   a) Waverly Assisted Living Facility, 9309 Mercy Way, Trinity,  FL  34655 - observed on or about 09/16/2022,  two employees in the process of installing foam board working from a Genie aerial lift, Model S-40, Serial number S40H-24891, were exposed to an approximate 14 foot fall hazard, in that restraint devices were not utilized.
