Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  a) On or about July 23, 2022, at the jobsite located at Rochell Road and Guadalupe Drive, Rockwall, TX, where scaffolds were not dismantled only under the supervision and direction of a competent person qualified in scaffold dismantling. The employer did not ensure that the activities were performed only by experienced and trained employee selected for such work by the competent person.

Employers were required to provide fall protection for employees erecting or dismantling supported scaffolds where the installation and use of such protection was feasible and did not create a greater hazard.  a) On or about July 23, 2022, at the jobsite located at Rochell Road and Guadalupe Drive, Rockwall, TX, where employee was dismantling supported scaffolds system without fall protection. The employer did not have a competent person determine the feasibility and safety of providing fall protection for employee dismantling supported scaffolds.
