29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a). On or about September 20, 2022 six employees were stripping old roofing and flashing from the second floor of a two-story condominium unit and were not protected by any means of personal fall protection, guardrails, or safety nets exposing them to a falls in excess of 19 feet to the ground below.

29 CFR  1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.   a) Jobsite: Employees were using a RIGID brand 2hp, 30-gallon air compressor connected to a flexible power cord that did not maintain a ground connector pin, exposing employees to electrical shock hazards, on or about September 22, 2022.  b) Jobsite:  Employees were using a flexible temporary cord to energize the circular saw on the roof was damaged, causing the cord to be plugged in to an energized cord exposing the employee to electrical shock hazards, on or about September 22, 2022.
