29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:  a) DJW Services Corporation, Inc dba Touching Hearts at Home: On or about September 21, 2022, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2022: A work-related death occurred on August 6, 2022, where an employee providing care at a residence suffered fatal injuries from a gunshot wound inflicted by the client.

29 CFR  1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident:  DJW Services Corporation, Inc dba Touching Hearts at Home   a) Jobsite located 1012 Kaolin Road, Kennett Square, PA 19348: On or about August 6, 2022, an employee working as a caregiver in a client residence suffered fatal injuries from gunshot wounds inflicted by the client. The employer did not report the work-related fatality to OSHA within 8 hours.
