Located at the vicinity of 7403 Summerfield Drive, Lewis Center, Ohio: On or about September 23, 2022, the employer did not ensure that employees performing sanitary installation work onsite were trained on the recognition and avoidance of hazardous conditions while performing trenching work, such as but not limited to, working in close proximity to energized electrical lines and trenching and excavation safety requirements in accordance with 29 CFR 1926 Subpart P.

Protection shall be provided by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  a. Located at the vicinity of 7403 Summerfield Drive, Lewis Center, Ohio: On or about September 23, 2022, the employer did not ensure that the spoil pile was located at least two feet from the edge of the excavation when two employees conducted potholing work, thereby exposing the employees to soils falling into the excavation
