29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets:  a. At a temporary worksite, located at 223 Mill Creek Road,  Jacksonville, Florida 32210: On or about September 27, 2022, the employer exposed an employee to head injury hazards caused by flying objects/debris, in that head protection was not utilized by the employee working beneath another employee, while picking up tree branches.

At 223 Mill Creek Road, in Jacksonville, Florida 32210: On or about September 27, 2022, the employer exposed an employee to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while using a Stihl chain saw to trim trees.

At 223 Mill Creek Road in Jacksonville, Florida 32210: On or about September 27, 2022, the employer exposed an employee to fall hazards ranging from 6 to 15-feet, in that fall protection, such as, but not limited to, safety harnesses with lanyard attached to the boom were not utilized by the employee, while installing trimming tree branches.
