29 CFR 1910.134(e)(1): The employer shall provide a medical evaluation to determine the employee's ability to use a respirator before the employee is fit tested or required to use the respirator in the workplace. The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator. On or about September 28, 2022, at location 1600 Ely Street, Marinette Wisconsin: the employer did not provide medical evaluations to determine each employee's abilities to use respirators, exposing employees to the hazards associated with inadequate physical/physiological capability. Employees were using half-face elastomeric respirators to protect from respiratory hazards while performing work such as welding in confined areas.

29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. On or about August 8, 2022, the employer did not ensure that case number 22-32 on the employer's OSHA 300 form for 2022 contained a complete description of the object/substance that directly injured or made the person ill.
