29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  (a)  On or about August 16, 2022, employees were exposed to injuries from falls, while working on a mezzanine being installed in a commercial building, with a section, approximately five foot by five foot, missing, resulting in one employee suffering serious injuries resulting from falling through the hole approximately 16 feet to the lower walking working surface.

29 CFR  1904.39(a)(2):The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:  (a)  On or about September 7, 2022, and times prior thereto, the employer failed to report a work related employee injury resulting in hospitalization in excess of 24 hours.  The hospitalization occurred on August 16, 2022 and did not report the incident to OSHA until September 6, 2022.
